New Patient Registration

If you wish to register at the practice please ask at reception . We currently have a waiting list due to high demand. We are happy to accept anyone living in our practice area onto the waiting list. When you reach the top of the waiting list you will be contacted by the practice manager and invited to join the list if you still wish to.

When we accept your application to register you will be offered a “New Patient Registration Check” with the practice nurse.

All new patients will be offered a health check with the practice nurse when they come to register. This is a chance for you to make sure you are up to date with routine immunisations and to have a basic health screen performed.

When you attend please bring a sample of urine with you and bring any repeat medications or repeat slip.

We define our practice area as Wootton, Wootton Fields and Hardingstone and Quinton. However, depending upon current demand, it may be necessary to decline applications to register but to join the waiting list.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
